Department of Justice 160-hour Jail Officer Certification TrackThe following records
must be on file for each College Jail Officer Certification Track student.
- Student Authorization for Release of Information
- Completed copy of Application for Employment as Law Enforcement, Jail or
Secure Detention Officer form
- Record of oral interview
- Results of criminal history records check
- Completed Physician's Assessment Form providing authorization for
participation in training
- Birth Certificate
- Verification that the student has a high school diploma or has passed
the general education development test or any other test recommended by the
Wisconsin Department of Public Instruction indicating high school diploma
level
- Documentation of attendance
- Documentation of any disciplinary or other problems involving students
and sanctions imposed
- Examination results
- Completed skills competency checklists and results
- Performance assessment task results
- Student form attesting that the school's written rules, penalties and
grievance procedures have been distributed, read and understood
- Student signed form attesting that the school's grading policy has been
distributed, read and understood
- Copies of student's transcripts
