Department of Justice 160-hour Jail Officer Certification Track

The following records must be on file for each College Jail Officer Certification Track student.

Student Authorization for Release of Information
Completed copy of Application for Employment as Law Enforcement, Jail or Secure Detention Officer form
Record of oral interview
Results of criminal history records check
Completed Physician's Assessment Form providing authorization for participation in training
Birth Certificate
Verification that the student has a high school diploma or has passed the general education development test or any other test recommended by the Wisconsin Department of Public Instruction indicating high school diploma level
Documentation of attendance
Documentation of any disciplinary or other problems involving students and sanctions imposed
Examination results
Completed skills competency checklists and results
Performance assessment task results
Student form attesting that the school's written rules, penalties and grievance procedures have been distributed, read and understood
Student signed form attesting that the school's grading policy has been distributed, read and understood
Copies of student's transcripts
