Get Licensed

Steps to apply for and complete the process to become a licensed EMT-Intermediate Technician:

• Complete an initial application
• Complete the Verification of Out-Of-State Education form and Verification of Out-Of-State Licensure
• Submit a copy of the Certificate of Completion
• Be prepared to submit:
   
  • A copy of both sides of your current CPR for Healthcare Providers certification card
  • A copy of the Certificate of Completion for your most recent EMT-Intermediate Technician refresher course

Materials will be reviewed and applicants notified of eligibility.
