LPN/RN Application Information

NCLEX APPLICATION (administered by Pearson VUE):

To register for the NCLEX examination you must go online to http://home.pearsonvue.com/.  Click on "For Test Takers/Schedule an Exam" and type in NCLEX to get started.

Before you can take the NCLEX, you'll need an Authorization to Test (ATT).  To get this, you'll need to apply to your state board of nursing/regulatory body and then register with Pearson VUE.  You'll want to start this process well in advance of your target date for taking the exam. 

  1. Complete all items as requested
  2. PN Program Code:  US50100300 
  3. RN Program Code:  US50407200
  4. Pay using a credit, debit or prepaid card (VISA, MasterCard, or American Express only)
  5. NCLEX is a national exam with the following test sites:  https://www.ncsbn.org/testing-locations.htm

WISCONSIN LICENSURE APPLICATION (Wisconsin Dept. of Safety & Professional Services):

If a 1st time test taker, apply Online thru OLAS (Online Licensure Application System):
LPN:  BE SURE TO CHOOSE Other (LPN) as your degree https://dsps.wi.gov/Pages/Professions/LPN/Default.aspx

Online Licensure Application System (OLAS) is only for initial nursing applicants
still needing to sit for NCLEX.

If you’ve previously sat for NCLEX and now want to be licensed in WI,
you MUST complete the paper application.

  1. When you get to the OLAS page, click on Help.  There is an Applicant FAQ and Applicant Manual to help you get started!
  2. Complete the Registered Nurse/Licensed Practical Nurse Licensure by Examination Application as applicable
  3. Nursing School:  WISCONSIN INDIANHEAD TECHNICAL COLLEGE
  4. School Address:  SHELL LAKE WI  USA (not campus location)  Location MAY show as Rice Lake, WI.  Choose whichever location appears
  5. December 2017 Graduation or Completion of Program Date:  December 21, 2017 (If graduating other then December 2017, the RN confer date or PN Milestone can be found by running an unofficial transcript through MyWITC)
  6. Type of Degree/Program:  RN (ADN) or Other (LPN) - completed portion of RN program needed to obtain a certificate of completion in PN

  7. Pay appropriate fees
  8. The state application is valid for 1 year.

If you are a new NCLEX applicant and you completed the online application through OLAS, the Statement of Graduation or Completion will be approved online.  DO NOT submit a paper copy of the Statement of Graduation or Completion if you completed your application through OLAS.

If you completed a paper application, complete & signed statements for RN graduates or students completing PN requirements on December 21, 2017, should be given to your instructor to be mailed to Shell Lake as a group.

  1. Complete ONLY the APPLICANT section of form
  2. X Registered Nurse   OR   Licensed Practical Nurse (as applicable)
  3. WITC will complete the WI BOARD-APPROVED SCHOOL section

MINNESOTA LICENSURE APPLICATION:

Confirmation of Program Completion form must be submitted in paper to Lisa Hall (see contact information below)

  1. Complete the online RN (or LPN) Licensure by Examination Application as applicable
  2. Name of School of Nursing:  WISCONSIN INDIANHEAD TECHNICAL COLLEGE
  3. Program Code:  LPN-US50100300; RN Code-US50407200
  4. City & State of School of Nursing:  SHELL LAKE WI  USA (not campus location).  Location MAY show as Rice Lake, WI.  Choose whichever location appears
  5. “X” Yes:  I authorize the release of my exam results to my school of nursing
  6. December 2017 Graduation/Confer Date:  X Yes, December 21, 2017 (If graduating other then December 2017, the RN confer date or PN Milestone can be found by running an unofficial transcript through MyWITC)
  7. Pay appropriate fees
  8. Complete paper form of Confirmation of Program Completion for Licensure by Examination (last page of the Application) & submit to Lisa Hall (contact information below)

All MN applicants MUST complete the Confirmation of Program Completion form in paper form.

  1. Complete only APPLICANT INFORMATION section of form
  2. Completion Date: December 21, 2017 (If graduating other then December 2017, the RN confer date or PN Milestone can be found by running an unofficial transcript through MyWITC)

All other WITC graduates/licensure applicants should send the completed Statement of Graduation or Completion form (for Wisconsin) or Confirmation of Program Completion form (for Minnesota) to:

If you've completed an application thru WI OLAS, new applications are checked weekly and will be approved at that time.
