Background Checks
All TM students are required to submit to a Wisconsin Caregiver Background Check at the beginning of the program.  A felony and some other violations may prohibit completion of the program and may prevent a student from taking the national and/or state exam. The cost of the background check is the responsibility of the student.  Th statement of understanding describes this in more detail.

Student Biographical Change Request -- please print and use this form if you change your name, address or phone number(s). You should submit it to your local Student Services to be certain that the changes are entered in the main computer system.
